Prevention of adverse events through planning and teaching the patient or family for hospital discharge.
Abstract
It is a reality not only nationally but globally, that high morbidity rates predispose the individual to be admitted to hospital units, and during this process it is necessary to plan and teach the family member and/or patient the care that they will require at home for of maintaining health or recovering it or rehabilitation. The objective of this research is to identify the continuity of care at the patient's discharge with the discharge plans. This is an observational and descriptive study with a sample of 150 adults and children from five public health institutions. A verification checklist was applied from hospital discharge to discharge, with a clinical data section.
