Abstract

The study analyzed the impact of the family record on the family and community health care model in Ecuador, with the aim of evaluating how this tool influences health care, the personalization of care, and the continuity of follow-up. An exploratory research with a qualitative approach, case analysis design was carried out at the "El Carmen" health center in Manabí through semi-structured interviews with family doctors, and a content analysis based on theoretical saturation was applied. The results highlighted the usefulness of the family record in the management of chronic diseases, identifying risk factors and more precise follow-up, being a valuable resource that improves the quality of care by offering a comprehensive view of the patient and their social environment.
